“What a beautiful morning,” you think as you stroll through the parking lot of your hospital. Feeling that the day is off to a great start, you walk in through the automatic doors—only to be greeted by a scene of pandemonium.

Before you have time to regain your senses, your nurse rushes at you, slaps a medical record into your hands, and taking you by the arm, vigorously guides you into your clinic.
